Open your ballot and mark your vote. If you do not want to be represented by a union, Vote NO. DO NOT SIGN OR DEFACE YOUR BALLOT!

Place your marked ballot in the blue envelope. DO NOT SIGN THE BLUE ENVELOPE!

Place the blue envelope inside the yellow envelope that is addressed to the NLRB. SIGN THE YELLOW ENVELOPE!
4.
Mail yellow envelope to the NLRB (No postage necessary).
Important Reminders
Voters must sign the outside of the yellow envelope. Any ballot received in an envelope that is not signed will be automatically void. DO NOT SIGN YOUR ACTUAL BALLOT OR THE BLUE ENVELOPE TO ENSURE YOUR PRIVACY IS PROTECTED
Mail Your Ballot No Later Than May 15th To Ensure The NLRB Receives it by May 26th.
